Steps to becoming a KidMed kid
1. Call our office to get registered

Our team will gather a few basic details and help schedule your child’s first visit.
2. Transfer your child’s medical records
To provide the best care possible, we like to review your child’s previous medical records.
Simply contact your child’s previous pediatric office and ask them to send the records to KidMed. Most offices will ask you to complete a records release form.
You can download our form below and send it directly to your previous provider.
3. Confirm KidMed with your insurance company
Once your appointment is scheduled, contact your insurance company and make sure KidMed is listed as your child’s primary care provider.
