Steps to Becoming a KidMed Kid

1. Get Registered!
Call our office at 252.364.4690 and speak to one of our staff members who can ask a series of questions and get you established as a new patient. You can also make an appointment at this time!
Once you receive an invitation to the patient portal, fill out any paperwork requested right from the comfort of your own home – this means less time in the waiting room!
2. Transfer Your Child’s Medical Records
Because we want to be comprehensive in the care we provide to your child, we’d like to have his or her medical history. You can call your child’s previous healthcare office and request that the records be transferred to us, but often they will ask you to fill out a form. There is a form here that you can use.
- Download and print one copy of the form for each child. Unfortunately, each child MUST have a separate form.
- In the top part, fill out the information of the place from which you are going to request the records. You will likely have to call them to get an email address or fax number to send it to. All of that information goes at the top.
- In the middle part, fill out the information about your child.
- If you do not know your child’s medical record number and you did not request it when you called, it may be okay to send it in anyway. Having that information, however, makes it easier for the medical records team at your previous location.
- If you do not know exact dates of service, you can simply write “all dates of service”
- Then you must sign the form.
- Finally, you fax or email or drop off the form to their office. Our information is already included on the form and they can send the records directly to us!
3. Check With Your Insurance Company
Finally, you will want to contact your insurance company (either by phone or online) and ensure that they list KidMed as your primary care provider. Be sure that they check the address to ensure they have the right place. We are at 1165 Cedar Point Boulevard, Suite F in Cedar Point, NC.
This is extremely important, as insurance companies may deny charges if a different primary care provider is listed. We don’t want our patients to have to pay for services their insurance denied because of a forgotten phone call! Their contact information should be located on the back of your insurance card.
